2023 Registration Instructions – DEADLINE AUGUST 15.
Registration is done via postal mail and is first-come first-serve. Travel league team placement cannot be guaranteed so get your registration in early. The teams (max 12-14 girls per team) do close out every year and then a wait list begins. Please follow all instructions carefully and fill out all forms completely. Incomplete registrations will be returned and could cause you to lose a spot on a team.

1. Registration Form.
Grade is as of Fall 2023.
3rd, 4th, 5th and 6th Graders only: Complete and sign the registration form (link found on left). NON-REFUNDABLE fee is $200 per player if received by August 15. $250 after August 15 – no exceptions!
**If an out of district team (non Northport-East Northport School district) does not have a parent volunteer coach, an additional $50 per player fee will be issued to cover the cost of paying a coach.
2. Insurance.
All athletes are required to hold current USA Field Hockey membership for insurance purposes. The non-refundable cost is approx. $25.00 annually. Click the link on the left to join. Select “U12 Membership” and follow the instructions. Please register in the athletes name – not the parents name. In the “Club” drop-down you should select “Long Island Field Hockey Association” or if you also play for another club team you can select that. If this athlete has already had this membership either current or expired, you should click the link to login to your account and simply “renew” the membership. Please print out the membership card showing valid current dates including the time period of at least Aug-Nov 2023. We do not want your receipt – just the membership card please (sample below). **Do not forget to include a copy of membership card with your registration packet .**

3. Mail.
Mail your completed and signed registration form, a copy of your USA Field Hockey membership card showing valid current dates, a copy of the player’s birth certificate or proof of age (new players only) and your check payable to “Northport Field Hockey Club”.
The mailing address is at the bottom of the registration form. Team placement may not be complete until beginning of September. If you have any specific team/coach/car pool requests you must indicate that on your registration form. We may not be able to honor such requests made after the fact. Schedules will not be available until early-mid September and games do not start until late September.
Please make sure you follow the above easy instructions closely. Your cashed check is proof we have received your registration.
